- Visit the landing page for the Hospital / Community Patient Referral Instrument (H/C PRI) training.
- Click on the “Enroll in training” button.
- You will be taken to the PHTC Online log in page:
- If you have an account on PHTC Online, enter your username and password and click the “Log In” button.
- Otherwise, click the button to “Create a PHTC Online account,” fill in the requested information, and click the “Create Account” button.
- Review the information presented. Click the button at the bottom of the page to proceed to the next step.
- Review the information on the Important Checks page. Check the boxes to indicate that everything is correct, and select your RN licensing status, which if and how you can be certified as an H/C PRI Assessor. When you are done, submit the information using the button at the bottom of the page.
- Select your Occupation and Affiliation. If you indicated you are a NYS-licensed RN, you will also need to enter your RN License number for automatic verification. Click the button to proceed.
- Submit your payment:
- To pay via credit card, click the “Pay via Credit Card” button and follow the prompts.
- To pay via any other method, enter the Transaction ID you received from the School of Public Health in the Transaction ID field. Then click the “Submit and Begin the Training” button.
- Once payment is complete, you will be taken to the main menu, where you can begin reviewing the training's content.
