Assess a person for possible mental illness, intellectual disability, or developmental disability with the Department of Health's SCREEN form. A certification card and number will be issued to discharge professionals who complete the training.

Hospital and Community Patient Review Instrument (H/C PRI)
Registered Nurses in New York use the H/C PRI to assess a person's condition and the amount of care required. RNs who complete this training will be certified to use the H/C PRI and issued an Assessor Identification number.
